POSITION SUMMARY: The Bereavement Care Coordinator has responsibility for supporting Bereavement Care services by working collaboratively with Bereavement Care Team as well as other SSRCH Disciplines.
Responsibilities primarily include performing administrative assistant duties, assisting, and implementing some aspects of bereavement care services, participation in planning and implementing events, and supporting community education.
ESSENTIAL RESPONSIBILITIES Administrative Assistance Assist with our Electronic Medical Records System (Wellsky).
Research and rectify bereavement data entry issues.
Assist in charting interventions and discharges.
Run care-planning and intervention reports.
Oversee and identify discrepancies in bereavement EMR Reports and relay issues to Bereavement Facilitator and/or manager as needed.
Assist in maintaining the proper EMR settings to reflect individualized bereavement plan of care.
Bereavement Care Oversee & execute bereavement mailings and phone calls.
Schedule and coordinate bereavement volunteers to assist with mailings and phone calls.
Work with receptionist on all aspects of bereavement spreadsheets required for bulk mailings.
Keep inventory of bereavement mailing supplies including 1st and 3rd party literature.
Assist & execute bereavement events, which may include evening & weekend work.
Serve as event planner for bereavement memorials and other special bereavement events, such as but not limited to: Memorial Services, Grief Through the Holiday, Butterfly Release and Reflection.
Work with receptionist to coordinate guest lists and RSVPs.
Co-facilitate Cup of Hope Support Group with potential to facilitate support groups in the future.
Assist with the development of bereavement education, program offerings and Cup of Hope curriculum.
Areas of Growth Assist with program archiving and sustainability planning.
Assist with bereavement reflection events for hospice personnel.
Attend IDG and serve as a liaison to the program (depending on need).
Offer early bereavement support/activities in IPU (depending on qualifications).
PROFESSIONAL QUALIFICATIONS Bachelor’s degree in social services or related field required; master’s degree preferred.
Accredited Certification in Grief and Bereavement Counseling preferred.
Experience in hospice or palliative care, healthcare, or non-profit required.
Proficiency in the use of Microsoft Office (Excel, Access, PowerPoint, and Word) or similar software required.
Strong public speaking/presentation skills & excellent verbal and written communication skills required.
Possess strong problem-solving and critical thinking skills with the ability to analyze data.
Maintain working knowledge and continued education in the field of death, dying and grief and bereavement.
WORKING CONDITIONS Mental Requirements: Read, speak, write, and understand English; cognitive ability to handle complex business decisions.
Environmental Demands: Work is primarily performed in the organization’s offices.
Occasional travel required.
All employees driving for hospice business are required to have a valid driver’s license, proof of current auto insurance, and maintain ability to pass background checks.
Fast-paced changing environment, with multiple requests from several disciplines.
Physical Requirements: Manual dexterity to operate computer and other office equipment.
Able to sit for extended periods of time.
Ability to have hearing and vision correctable to normal levels.
Ability to locate and drive to and from facilities and to meetings at other locations as necessary.
Ability to ambulate on a frequent basis, constant bending, lifting, and twisting.
Monday - Friday, 1st Shift with occasional nights and weekends as needed.
Part-time, 20 hours per week
