Case Managers in the CAT program provide assessment and recovery focused services for individuals and families who may have mental health needs.
As part of the team, Crisis Case Managers participate in clinical staffing, treatment recommendations, information and referral inquiries, crisis intervention, and provide consultation within the agency, to community service partners and the public.
The primary function of this position is to provide in-depth assessments, short term case management and appropriate referrals to meet identified needs.
*Essential Duties* -Completes comprehensive initial assessments that reflect the client and families need for mental health services to include presenting problems, psychosocial history, diagnostic impressions, functional screens, and recommendations for treatment.
- Provides results of the assessment to the client and family as well as treatment alternatives, client’s rights and responsibilities, the anticipated fees for treatment, and the clinic’s grievance procedure.
- Develops a treatment or referral plan in collaboration with the family based on the initial comprehensive assessment to include strengths, treatment goals, measurable outcomes, the frequency and type of service recommended to reach goals.
- Conducts follow up with the family to evaluate progress toward goals, the need for additional assessment or changes in the plan.
- Provides crisis intervention to open families which may include development of a crisis plan and/or collaboration with our crisis system service partners in the community.
- Maintains client’s electronic record in accordance with confidentiality rules and regulatory requirements and standards.
Timely completion of documentation is required to include the initial (and subsequent) assessments, progress notes, staffing/supervision meetings, collegial clinical collaboration, treatment plans and reviews, collateral contacts, phone calls/texts/emails, and a closing/discharge summary.
- Monitors need for and complete any documentation/forms required by third party payers to support reimbursement of services provided.
Provide accurate, adequate documentation in electronic record to support billing and reimbursement of services rendered.
- Performs short term, case management functions for open families.
- Provides information and referral to individuals about community mental health and addiction resources; consults with agencies, schools, and other health professionals as needed and with consent concerning assessments, treatment planning, and recommendations.
- Promotes recovery and assures the involvement of families, and/or other natural supports whenever possible with client consent.
- Provides outreach and/or crisis services in situations where a client may be experiencing difficulty to facilitate his/her entry into an appropriate level of services.
- Participates in administrative and other duties as assigned which may include being a member of a committee, reviewing policies and procedures, maintaining spreadsheets, or providing support and input into the planning of new programs and in-service trainings.
- Performs other duties within the scope of professional responsibilities as assigned by the supervisor.

*Qualifications* - A masters degree in a behavioral science related field and 1,000 hours of supervised post- degree clinical experience serving individuals with severe and persistent mental illness.
- Knowledge and understanding of federal laws, State statutes, and administrative rules which regulate the services to be provided.
- Knowledge of and sensitivity to social problems, cultural diversity, human growth and development, and individual and family functioning.
- Ability to coordinate activities and functions that cross unit and departmental lines and involve other county and community agencies.
- Ability to follow instructions, to complete tasks, and to work closely with the supervisor and others.
- Strong knowledge base in recovery principles, person-centered planning, and trauma informed care.
- Excellent communication skills (oral and written), aptitude for critical thinking, good time management with ability to effectively prioritize responsibilities, and high level of professionalism and ethical practice.
- Positive attitude, enthusiasm, and sincere conviction for providing services to clients with chronic mental illness and/or addiction with complicated psychosocial circumstances.
- Ability to establish positive connections and develop relationships with clients, their natural supports, and other professionals.
Able to meet each client where they are and instill hope for recovery.
- Ability to maintain a calm demeanor and make good clinical decisions while adhering to clinical, ethical, and regulatory standards and comfortable seeking consultation when needed.
- Capable of demonstrating a high level of professionalism and a positive attitude in the community as a representative of Sheboygan County Health and Human Services.
- Ability to provide excellent customer service.
- Unlimited access to a car, valid driver’s license, and ability to drive or arrange travel in and out-of-county, as requested by supervisor or his/her designee.
*Work Environment & Physical Requirements* Regular contact with clients and their family/natural supports, Behavioral Health Program clinicians and case managers, representatives from our crisis services continuum, other SCHHS staff, community service partners, school personnel, and members of the public.
May occasionally work with aggressive or violent individuals.
Work is performed in a standard office setting, in homes, and in schools, with occasional need to provide services in other locations.
There may be travel to a variety of community and/or regional locations.
Must be able to operate standard office equipment including telephone, computer, facsimile, calculator, printer, and photocopier.
Ability to lift up to 20 pounds and a combination of sitting and standing postures may be required.
Risk of blood borne pathogen exposure is considered low.
Job Type: Full-time Pay: $27.
27 - $35.
